Thank you for choosing  our office for your endodontic needs.

The fee for your endodontic treatment will be based on the extent of your treatment. After your consultation/evaluation, the fees involved will be discussed. 

We are currently not in any dental insurance network and do not take assignment of benefits. It is our policy that your care is paid in full at the time of treatment.  

We will assist you in processing your insurance claim and submit it on your behalf and provide you with all the information required by your insurance carrier so that you may obtain any reimbursements due you. Please provide your insurance information accurately.

We accept payment by cash, check, debit and credit cards.  Credit card companies charge a convenience fee of 3%, which will be assessed to you.  To avoid this fee, please be prepared to make your payment with cash, check, or debit card.

Treating your endodontic needs promptly is our utmost goal.  We strive to schedule your appointment as quickly as possible. With the high demand for quality endodontic care and patients with emergency needs, we kindly request a 48 hour business day notification if you need to reschedule or cancel.  Please keep in mind our business days are Monday through Thursday by 5 pm. 

We understand that true emergencies do occur  and we will be understanding, however, if you do not give us notice or fail to show for your confirmed appointment, we reserve the right to charge a broken appointment fee of a $100.  Please help us to serve you better by keeping your scheduled appointment. 

With multiple broken appointments, we reserve the right to release you as a patient.
